One man has died and another was seriously injured last Thursday following an industrial accident at ATI Ladish Forging in Cudahy, Wisconsin.
Through operations in Wisconsin, California, Connecticut, Indiana and Kentucky in the U.S. and Stalowa Wola in Poland, ATI spans materials from carbon and alloy steel to nickel/cobalt alloys, powder metals, titanium and other specialty metals.
The company employs a wide range of forging technologies, including isothermal and hot-die forging for advanced aerospace materials and components. And ATI provides a full range of post-forging inspection, machining and finishing services.
A spokesperson for Ladish confirmed the death, according to a report by the Milwaukee Biz Times , but declined to offer specifics.
“The Milwaukee County medical examiner responded to the accident around 2:30 p.m. A 48-year-old Cudahy man has been hospitalized, according to WISN 12,” an article by the Oak Creek Patch reads. “Ladish makes engineered metal components for jet engines, aerospace and industrial markets. The Biz Times is reporting that an OSHA investigator was at the site conducting an investigation alongside authorities. OSHA has cited Ladish for 13 safety violations since 2007.”
The article does not mention what type of accident happened or give details about the survivor’s status.
A search on OSHA’s website shows the ATI Wisconsin location had a serious violation in 2015 and was penalized $7,000. The site does not reveal what the violation was other than that it was a safety violation.
There were two OSHA reports made in 2012, one was a complaint, which OSHA recognized as a serious safety hazard and fined the Wisconsin branch $8,500. The website shows three issues were settled in that case. The other report was a referral that found the branch to have three serious safety violations. The branch was fined $11,000 initially. The case was settled informally, according to the OSHA website.
